Please check the schedule's link on our site before submitting an appointment request. Be sure the lady your requesting is listed for the day and time frame of your appointment request.

If you are already verified or have used our service already there is no need to read any further.

If you are a new client please fill out the appointments form completely. In the comments field list that you are a new client and need to get verified.

Verification:

There are several options for getting verified, you can select one or any combination.

1: Provider references: If you have seen any other services or independent please list them in the comments field. Note: If using provider references it must be from a credible and known provider. If you have multiple references please list them for it speeds up the verification process.

2: Board handles: If you use TER, P411, TBD or DATECHECK please list this. Note: there must be activity on the account.

3: Employment verification: We are very discreet in this process. We will not contact your employer or do any thing that would implicate your career. If you select this as your verification method we would need the following: Your real first and last name as it appears on your ID. The name of your company, the city and state your company is located in. The main phone number to your company. If your company has several locations we would need which branch you work at and your position in that company. (If you have solid references and board handles the employment verification is not needed.)
